About the Database

The National Chronic Disease Self-Management (CDSME) Database was established in 2010 through support from the Administration for Community Living (ACL). The Database, housed at and managed by the National CDSME Resource Center at the National Council on Aging (NCOA), has grown significantly over the past decade. The database receives data from active ACL Prevention and Public Health Fund (PPHF) CDSME grantees, former grantees, and other organizations interested in tracking data for their evidence-based health promotion and disease prevention programs.
